Consent to the Processing of Personal Data

I (hereinafter referred to as the "Data Subject"), hereby provide my Consent to the processing of my personal data (hereinafter referred to as the "Consent") under the terms set forth below. This Consent is provided by checking the corresponding box and clicking the "Submit" button when filling out the form on the Operator's website at https://mbpharma.ru/ . Acting freely, according to my own will and in my own interest, and confirming my legal capacity, the Data Subject provides consent to Joint-Stock Company "MiraxBioPharma" (INN 7730532561) (hereinafter referred to as the "Operator") to process my personal data in accordance with the following terms:

This Consent is given for the processing of the following personal data, or part thereof, by the Data Subject, which are not special or biometric:

• Last name, first name, patronymic;
• Email address;
• Residential address;
• Contact details (phone number);
• Other information about the Data Subject provided to the Operator in connection with the Data Subject's inquiries to the Operator.

The Operator may use the industry-standard "cookies" technology. Cookies are a small piece of data sent by a web server and stored on the computer used by the Data Subject, allowing the Operator to save the Data Subject's personal settings and preferences, as well as to collect non-personal information about them.

The processing of personal data by the Operator means any action (operation) or a set of actions (operations) performed with personal data, including collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, and destruction of personal data.

The Data Subject gives consent to the processing of personal data for the following purposes:

Identification of the Data Subject;

• Interaction with the Data Subject, including sending notifications, inquiries, and information related to the Operator's actions, as well as processing requests and applications from the Data Subject;
• Processing reports of adverse drug reactions within the framework of pharmacovigilance;
• Ensuring the quality operation of the Operator's website;
• Ensuring interaction between the Data Subject and the Operator for the purpose of providing services or performing work for the Data Subject;
• Achieving the purposes stipulated by law for the Operator.

The Data Subject has the right to send the Operator a request to clarify their personal data, a demand to block or destroy them if the personal data are incomplete, outdated, or inaccurate. This Consent may be withdrawn subject to notification sent no less than 30 days prior to the intended date of termination of data processing by the Operator. All inquiries regarding the processing of personal data may be sent by the User via email to the Operator's address: info@mbpharma.ru. The Data Subject consents to the transfer of their personal data by the Operator to third parties for the provision of services to the Data Subject in accordance with their request on the Operator's website, or for other similar performance of the agreement between the Data Subject and the Operator.

The Operator processes the Data Subject's personal data both with the use of automation tools and without them, in accordance with the regulatory legal acts of the Russian Federation that establish requirements for ensuring the security of personal data during their processing and for observing the rights of personal data subjects. The use, clarification, distribution, and destruction of the User's personal data are carried out by the Operator in accordance with the specifics approved by Decree of the Government of the Russian Federation No. 687 dated September 15, 2008.

The Operator takes necessary and sufficient organizational and technical measures to protect the Data Subject's personal information from unlawful or accidental access, destruction, modification, blocking, copying, distribution, as well as from other unlawful actions by third parties.

In the event of the withdrawal of this Consent to the processing of personal data by the Data Subject or their representative, the Operator has the right to continue processing personal data without the Data Subject's consent if there are grounds specified in clauses 2-11, part 1, article 6, and part 2, article 10 of the Federal Law No. 152-FZ "On Personal Data" dated July 27, 2006.
